Sugar vs Sweeteners: The 2023 WHO Guideline that changed the conversation

In May 2023, the World Health Organization issued a conditional recommendation against using non-nutritive sweeteners for weight control or disease prevention. The evidence review behind it found no long-term benefit for reducing body fat.

Because the certainty of evidence was rated low, WHO’s recommendation is conditional rather than absolute, and it explicitly excludes people who already have diabetes.

Aspartame’s cancer classification, explained

Two months later, the International Agency for Research on Cancer classified aspartame as “possibly carcinogenic to humans” (Group 2B) — a hazard classification based on limited evidence. At the same time, the joint FAO/WHO expert committee left the acceptable daily intake unchanged at 40 mg/kg body weight.

For a 70 kg adult, that threshold works out to roughly 9–14 cans of diet soda per day before exceeding it.

Erythritol and xylitol: an active scientific debate

Sugar alcohols marketed as “natural” alternatives have drawn fresh scrutiny. A 2023 Nature Medicine study linked higher plasma erythritol to a 1.80–2.21 times higher risk of major cardiovascular events, and lab work showed increased platelet clotting activity after erythritol intake, with elevated plasma levels persisting for more than two days after a single dose. A 2024 European Heart Journal study found a comparable pattern for xylitol (HR 1.57 for the highest tertile), with the same research group demonstrating enhanced platelet reactivity in follow-up experiments.

Why this isn’t the final word

Both studies were observational, conducted in patients already being evaluated for cardiac risk — not the general population — and other researchers have published formal critiques of the statistical modeling in the same journal, including questions about how the exposure thresholds were defined. The study authors themselves called for further research into long-term safety rather than presenting their findings as conclusive. This remains genuinely unresolved, which is exactly why it deserves careful framing rather than a scary headline treatment.

Where these sweeteners hide, and what looks safer

Erythritol and xylitol aren’t limited to diet soda — they’re common in keto-friendly baked goods, protein bars, sugar-free gum, and monk-fruit blends, precisely because they mimic sugar’s taste and texture better than most alternatives. If you rely on low-carb products, it’s worth checking labels rather than assuming “sugar-free” is a blanket reassurance. Stevia, meanwhile, looks comparatively reassuring: recent meta-analyses haven’t found clear adverse effects on blood glucose or HbA1c. The takeaway isn’t that all sweeteners are equally risky or equally safe — it’s that the category is genuinely heterogeneous.

What this means for you

The American Diabetes Association’s 2025 Standards of Care recommend nonnutritive sweeteners as a moderate, short-term substitute for sugar-sweetened products in people with or at risk for diabetes — while emphasizing that water remains the preferred choice and that sweeteners alone don’t produce weight loss without calorie reduction. In practice: occasional use is reasonable for most healthy adults, diabetes management can lean on short-term substitution deliberately, and anyone with cardiovascular disease should be more selective about sugar-alcohol-sweetened products specifically and loop in their physician.
